In order to carry out a full diagnostic assessment, background information about the learner will need to be gathered. Please download the documents below (5 in total) making sure to select the appropriate school questionnaire (i.e. primary or secondary school). Once these have been completed and returned, either by post or e-mail, you will be offered a selection of assessment dates to chose from.
